Someone explain this Mystery

Recently repaired a Galaxy 2547 with Voltage regulator over heating,,Popping mosfets etc..The regulator would get so hot while receiving it would burn your hand..Put in all new mosfets and regulator last 20 mins popped again..Well after many hours of thought i said self could it be a power supply issue?Well keep in mind it's showing 13.6 v on a fluke 27..Normal current draw.Nothing out of the ordinary checked out A-OK.I thought well i have tried everything around 3 sets of mosfets and a few sb827's i thought i have nothing to loose..Well i ran 13,8v from an external supply and Perfect cool as can be with good power,,So i took apart the mean well supply and sure enough black soot everywhere. Now i tested this supply several times with load and no load..How was it causing the issues i was having?Chime in with your thoughts.Could Ac be getting into the radio somehow?

Interesting, I will say the PS in the Galaxy really struggles to power the radio and it gets worse as any mods are done. I always wanted to get one used, remove the PS and in the space install a 100w amplifier and like most ham radios, run a external PS. My other idea was to find a computer PS that would fit inside, needed to be at least a 400w supply and do the dual final mod.

I for one have repaired Many radio's and i can honestly say i have Never seen anything as crazy as that and i kept the fluke inline during all tests No fluctuations off of 13.6v and around 6 amps at full load..I am still Puzzled by this.

windings and insulation are not foolproof, i have seen it, an old exspurt told me of his experiences,
the real reason could be surmized with enough brainfarting ,
